04/06/2020 – Morning & Afternoon shift – zoe ball

Shift Report:

Zoe and I arrived at Carla and Michael’s at 0915. Fela was still upstairs sat on the toilet. Fela had had a bowel movement.

Zoe helped with fela in the shower and he got dressed and was downstairs for 0940.


I had fela’s porridge ready for when he came down. Fela sat down and said prayer then began to put the toppings on his porridge. It took fela 30 minutes to finish his breakfast.


When fela finished his supplements and drinks, it was 1030. Zoe did the board with him before we left and fela got ready to leave.


We got back to fela’s house at 1040.

Zoe did a new board with him and he listened to music.


At 1140, Zoe cleaned his keloids and then fela asked for a head massage. Zoe gave him a head massage and did some Handle with fela using the spikey ball.


At 1200, fela went to the toilet. Claire arrived and Zoe left.


At 1220, fela had his lunch time supplement, then sat down ready for lunch. Lunch was ready at 1335. Fela said prayer and ate well with staff. When fela had finished, he began to get vocal and looked annoyed.


At 1310, fela went to sit in his chair and got very vocal and aggitated. Bouncing in his rocker, and hooting his own head. I asked fela to stop hitting himself while I was cleaning the floor and then gave him some space.


At 1335, fela began to quieten down. Claire cleaned his keloids and put a dressing on them as he had been picking them. Fela seemed happier after this.


At 1340, fela became very vocal but happy. So he then did some colouring with Claire for 20 minutes.


At 1405, fela got his shoes on, ready to go to mama and Michael’s. He went to the toilet first to pass urine. Then we left.


I dropped fela off then had to take Claire back to get her car. Then come back to Carla and Michael’s house. Fela was sitting in the lounge, watching 101 Dalmatians with a cup of tea.


Fela was messing with his left middle finger nail, it had a split in it which I was worried he would rip off and only half the nail would remain. So I filed fela’s nail flat and rounded it off so there was no gap to pick at.

Fela enjoyed this and asked me to carry on and do the rest. So I did.


Five minutes later. Fela then began to usher me away and pointed to the remote, he pointed at the mute. So I muted it. Fela then got up and messed with a few bits and bobs, before going to the stairs and asking for the toilet. I said, “go on then” and as he went up the stairs, I saw a wet patch on fela’s trousers. Fela had been incontinent of a small amount of urine. (I then realised he was asking me to pause the film so he could go to the toilet)


Fela passed urine on the loo and I gave him a change of clothes. Fela then came back downstairs, in time for snack.


1535 fela had a snack which took him 50 minutes to eat. At 1620 fela got his hat, coat and shoes on.


Emma arrived at 1630. We got back to fela’s house at 1640 and I did the board for fela and Emma went through it with him while I made dinner. Fela Chose to watch Little Women.


Fela had his supplement at 1735 and then dinner was ready at 1810. Fela said prayer with staff and ate nicely. He was finished at 1830. Fela took his bowl to the sink then went to sit down.

Fela watched the rest of his film which finished at 1940. Fela then had his keloids cleaned and a new dressing applied.

I read fela his social story again about sleeping at green pastures.


At 2000, fela had his snack and drink. At 2030 fela put his shoes on and went to the toilet, before leaving for Carla and Michael’s at 2045.

We dropped fela off and said goodbye. And left at 2100.

Arousal States – Start: 1 Baseline End: 2a – Vocal but happy Did his state change? Yes to 5 – Very Agitated Feal ha been baseline for most of the shift. He got agitated after lunch for around 20 minutes. And was very vocal happy when leaving for Carla and Michael's. Did he have a bowel movement? Yes Bristol score: Was Fela incontinent?: Yes Did he have vigorous exercise?: No – Fela had little sleep and has been slow in processing.
