09/12/2020 – Evening shift – zoe holmes

Shift Report:

Fela was relaxing on my arrival, I had handover and then said hello to Fela.

I then started the evening meal and Fela relaxed.

I set the table and Fela appeared happy, he then went to the toilet and became agitated, once calm Fela sat down fir his evening meal, we said prayer and Fela ate well, Fela was vocal at points while eating, after finishing desert Fela became agitated, this lasted a few minutes, Fela then eventually sat down and relaxed, we did the board and put a movie on and staff sat with Fela.

Fela had a phone call from Mama at 7.30pm and then he had his evening snack at 8pm, Fela continued watching his film.

At 8.50pm we encouraged Fela upstairs for his shower, due to being extremely tired, Fela became agitated after passing urine, this lasted for approximately 15min.

Once calmed I encouraged Fela to take his clothes off and get into the shower, we left Fela to have his shower, Fela requested five, when I went into him, I gave Fela five minutes and then he requested five again, so we gave Fela extra five minutes, I then did count down and turned the shower off, Fela became very agitated, please see arousal state, this lasted for 15 to 20 minutes.

Once calm Fela completed routine but very vocal and slow, we encouraged Fela into bed and then he started to smile, we said prayer and goodnight, music cd left on, Fela was in bed for 10pm, Fela was humming after.

Arousal States – Start: 1 Baseline End: 2a – Vocal Did his state change? Yes to 5 – Very Agitated Fela as been agigtated on and off throughout the shift, Fela became vocal while eating dinner and after he became agitated.

When upstairs Fela went to the toilet became agitated, once calm Fela got into the shower, after his shower Fela became very agitated, stamping, gutteral noises, clapping hands, sweating, this lasted for 15 minutes.
