09/12/2020 – Sleep shift – zoe holmes

Shift Report:

Fela was in bed for 10pm, he became very agitated at around 10.15pm, rocking very hard in his bed, this lasted a few minutes.

Fela went to sleep around 11.30pm and remained asleep until 5am.

I heard Fela at 5.30am and he went to the toilet and passed urine, Fela then returned to bed.

Fela didn’t go back to sleep.

I went into Fela at approximately 7.10am with Fela’s morning refreshments, Fela was full of smiles and had these really well.

I prompted Fela to the shower room and Fela passed urine and then sat down, Fela was vocal throughout, he then got into the shower.

After Fela had his shower he became agitated while drying himself, I left Fela to calm, once calm Fela sat on his bed and then became agitated while putting his cream on.

Fela as become agigtated throughout his routine, so I have given Fela lots of space.

Once downstairs Fela is still agitated and is now currently agitated.

I have noticed this morning that at the back of Fela’s Pj Bottoms there is blood stains, I have passed this over to staff.

Arousal States – Start: 1 Baseline End: 4 – Agitated Did his state change? Yes to 5 – Very Agitated Fela became agitated at 10.15pm, rocking very hard in his bed, this lasted a few minutes.

Throughout completing his morning routine, Fela as been vocal throughout and become agigtated on numerous occasions, stamping his feet, clapping his hands, clicking his fingers, gutteral noises, sweating, rocking. Did he have a bowel movement? No Bristol score: Was Fela incontinent?: No Did he have vigorous exercise?: –
