10/01/2020 – Evening shift – kuda wadesango

Shift Report:

Felas mood was changing time to time. Done the board with him for his evening activities and routine. Tea was prepared and served. Fela said a prayer and had his supplements and meal. We let him relax for a bit before we started mr motivator. Fela did well for a good 15minutes. Sat back down had his drink and later on his snack. Fela appeared to be going to the toilet frequently (almost every half hour). His urine appeared clear and was no strong odour when I checked. Fela was just going toilet without any prompts at the time. His now sat in the bath quietly, splashing.

Arousal States – Start: 1 Baseline End: 1 Baseline Did his state change? Yes to 4 – Agitated Felas mood was changing from time to time with a slight agitation whilst sat having his tea with jerking his head. When he was sat on the rocking chair, it was observed Fela clapping and stomping his feet on the floor but this was for a brief moment. Did he have a bowel movement? No Bristol score: Was Fela incontinent?: No Did he have vigorous exercise?: No – Did Mr motivator for almost 15minutes.
