10/08/2020 – Evening shift – zoe holmes

Shift Report:

When I arrived on my shift, I said hello to Fela and had handover.

At 5pm Fela requested a drink of juice, the board was done and Fela appeared happy.

At 5.30pm I gave Fela his evening supplements with some water and he took these very good. After Fela went to the toilet, Fela passed urine but after flushing the toilet became vocal, please see arousal state.

Once Fela came out of the bathroom he was very happy, we sat down for our dinner and Fela said prayer well and ate well, after Fela enjoyed his rice pudding, Fela then took his apron out and I encouraged Fela to remove his top, due to sweating so much, Fela sat on the sofa and I wiped Fela down with his towel.

At 7pm myself and Fela showed Alicia some handle techniques and Fela did a good job.

After we did walk at home, Fela refused at first but with some encouragement he did it and was happy while doing it, after Fela relaxed sitting in his rocking chair Fela requested for me to sit with him, this evening Fela as wanted alot if staff company which as been very nice.

At 8pm Fela had his evening snack and was full of smiles.

At 9pm I supported Fela with his bath routine.

A really lovely evening with a very happy Fela.

Yes to 2b – Vocal unhappy Fela became vocal while in the toilet, Fela passed urine and after flushing the toilet, Fela remained standing by the toilet being vocal and stamping feet from side to side, I encouraged Fela to sit on the toilet, Fela remained in the toilet for around 30min in total.
