15/10/2019 – Day shift – zoe holmes

Shift Report:

Fela was agigtated on my arrival, I said hello and then had handover, after I sat with Fela while he ate his breakfast, Fela as been agigtated through the morning, up until around noon time, he then calmed, after breakfast Fela sat in his rocking chair and was agigtated on and off, the board was done, Fela had his morning snack but was very slow eating this, it actually took Fela awhile before he decided to touch his snack, around noon time Fela calmed and we had lunch and he was happy, after lunch I played cards with Fela and then sat next to Fela on the sofa and gave him a head massage, Mama brought Fela’s foot massager down and he sat using it and he was that relaxed he began to fall asleep, we then got ready to go to Mama and Michael’s house, Fela was so happy about this, once there Fela signed to go to the toilet, Fela became vocal and then agitated while in the toilet, Fela calmed and I changed Fela’s wet clothes and encouraged Fela to wipe himself with his towel, Fela put dry clothes on and I then encouraged Fela to have a lie down and relax, on my departure Fela was sleeping on the red sofa upstairs at Mama and Michael’s house.

Arousal States – Start: 4 – Agitated End: 1 Baseline Did his state change? Yes to 5 – Very Agitated Fela as been agigtated through out the morning, stamping his feet, rocking hard, gutteral noises, sweating, clapping his hands, this was on and off and lasted until around noon time. Fela became agitated in the toilet at Mama's house when we arrived. Did he have a bowel movement? No Bristol score: Was Fela incontinent?: No Did he have vigorous exercise?: No – Why no exercise?Fela as been agigtated through the morning.
