18/03/2020 – Day shift – zoe holmes

Shift Report:

Fela was sat in the lounge on my arrival, I said hello and then had handover, Fela relaxed while staff did chores and then he had his morning snack, we then got ready to go out, we ran some errands and returned home, Fela had been incontinent and I think this caused his agitation, please see arousal state, after Fela calmed he came into the lounge for lunch, Fela said prayer well, Fela became agitated at lunch time, after lunch staff supported Fela to get changed, Fela then relaxed, Chloe supported Fela to lie on the sofa, after he rested Fela came into the staff room comunicating with staff, he then went into the bathroom and passed urine, Fela as sounded very croaky today so staff have spoken to Carla and we have taken Fela’s temperature and it is normal, we are to follow the Cold and Flu guide, which is next to the computer in the lounge, Staff have applied tiger balm to Fela’s chest, back, neck and forehead. Fela is currently sat on the toilet.

Yes to 5 – Very Agitated Fela became very agitated when we arrived home, Fela had been incontinent, so staff encouraged Fela to the bathroom, Fela got very agitated rocking hard, vocal, clapping hands, jumping up and down on the toilet, this lasted for 45 minutes.

Fela became agitated at lunch time, rocking hard and jumping up and down while sat on the chair, he then calmed.

After lunch staff changed Fela's cloths.

Fela as been vocal throughout the afternoon and agigtated on and off throughout the afternoon. Did he have a bowel movement? Yes Bristol score: Was Fela incontinent?: Yes Did he have vigorous exercise?: No – Agitated through the shift. Why no exercise?
