20/09/2019 – Day shift – zoe holmes

Shift Report:

When I arrived fory shift I had been previously informed that Fela had bitten Maxine, when I arrived I had handover from Michael and Maxine, Fela was in a high state of arousal, I spoke to Fela calmly and I put some music on, I encouraged Fela to remove his wet top and to dry himself with his towel, Fela’s agitation states states were going from baseline to very agigtated, I tried to prompt Fela to his rocking chair but he declined, Mama arrived and she calmed Fela, after we got all of Fela’s things together for an overnight stay at Mama and Michael’s house, Fela was calm and happy on our departure and was very happy to arrive at Green Pastures and to see Mama.

Arousal States – Start: 6 – Aggresive End: 2a – Vocal but happy Did his state change? Yes to 5 – Very Agitated When I arrived for my shift Fela was extremely agigtated, an incident had happened involving a member of staff, Fela had bitten staff, it took approximately two hours for Fela to calm properly, He would calm then instantly become agigtated. Did he have a bowel movement? No Bristol score: Was Fela incontinent?: No Did he have vigorous exercise?: No – Why no exercise?
