I arrived for my shift fela was still at mama and micheals. I waited for Dan at fela. On Dan’s arrival we left to go qbd collect fela from mama and micheals on our arrival fela was sitting in the armchair. Had handover from Carla. While in car fela was vocal and seemed to be very excited. On returning home fela took shoes of went sat in rocking chair and listened to music. Fela was vocal still but seemed excited beciase he was laughing and hissing.
Fela relaxed listening to music while I did some chores. Fela then sat down for his snack which he seemed to really enjoy. When fela had finsihed his snack he went sat in his rocking chair I asked fela if I could sit with him and he clearly told me to go.
Fela sat listneing to music. The district nurse then come to see fela she cleaned, redressed fela faces and filled in all paper work. Once she had finsihed fela relaxed listnend to music. I did the board with fela. Shortly after doing board fela became very nosiy. Fela went to toielt and past urine. Fela was very nosiy while in the batbroom
Fela then went to sit down in his rocking chair. Dan prompted fela to get his shoes on. We then left at 1 20pm to go on a neighbourhood walk. Fela walked for just over an hour and worked up a really good sweat . On retuong home fela took of his shoes and hat then went into bathroom and had complete clothing change. Fela then sat on the toielt asked for 5. When finsihed in bathroom fela became agaited clapping hands gruntal nosies, stamping feet, rocking backwards abd forwards virgosly. Fela dressing also come off but coukdbt re dressed yet due to fela begin agaiagrd This lasted for about 5 minutes. Fela then was up and down be happy to unhappy vocal . Fela was fully calm abd at baseline around 3.15.
At 3 30pm fela sat at the table for his snack. Once he had finsihed his snack. I re dressed fela face. After I did the board with fela.
Myself and fela did makaton we did summer, park, flower, garden ice lolly bird and sunshine fela remember these very well. Fela relaxed listening to music.
Teresa arrived for her shift gave handover. Fela sat listnening to music. While we did some chores .
I then did foot physio and handle with fela. Afterwards fela listned to music while did dinner.
We all then sat down for dinner fela said prayer and ate well. After dinner fela wiped down the table. Fela then went sat in his chair listened to music I did the bird with fela.
We then all got our shoes on and left to go on a neighbourhood walk we walked for about 45 minutes while on the walk fela hot slightly agaited stamping as he walked and jumping on spot this lasted for couple mintues then fela was fine. Fela was vocal on and off while on the walk on returing home fela took his shoes and jumper off, went sat in his chair and listened to music. He then sat at table for his snack and cuptea. Afterward fela went sat back in his rocking chair.
I then prompted fela in to the bathroom fela got undressed and into the bath I supported fela with his personal hygiene.
