26/03/2023 – Day & Evening shift – kingsley osei

Shift Report:

PN handed over when RT and I arrived for our shift, saw Fela seated on his settee listen to music, I said hello to him and updated the TEACCH board with him. He had the option of chosen between T.D Jakes and Breathe Church of which he chose Breathe church, changed his clothes and got him ready for church as he was vocal throughout. He started pointing to his forehead as a sign of headache of which I applied tiger balm to it. On our way to church Fela was quite but when we got to the entrance of the church he started rocking on his seat and was a bit agitated so RT and I had to get him away from the premises and the church donation (£2) has been added back to the petty cash and signed. Arrived a bit early to Green Pastures because of Fela’s behaviour and CC offered him Willow Bark capsules (2) to ease his headache. After lunch Fela has been agitated for sometime, when to the toilet to pass out urine and bowel and after that had his snack and took a nap at 16:00 and woke 45 minutes later. We returned from Green Pastures and handed over to AM, she prepared dinner and got Fela ready for dinning. Prayed, sat and ate, TEACCH board was updated to reflect the rest of activities, I gave another 2 capsules of William Bark as instructed by CC to give after dinner. Fela has been vocal and agitated since morning, 10 minutes of foot physio done with him as he declined walk with Leslie and requested for facial trim. Prepared and assisted him with his bath, at the time of my report Fela was rinsing himself in the tub.
