I arrived for  day and evening shifts and greeted Fela, who was calm and settled in the lounge. Carla was contacted by phone and after enquiring about how Fela was doing, she provided advice for the day. A social story about visiting Brampton Park was printed and read to Fela. We then got ready to go out.  Due to the rain, we decided to go for a drive instead. Later, the plan was to have lunch at Toby Carvery. I showed Fela a picture of Toby Carvery and explained that, because it was raining, we could not go to the park and would instead be going to Toby Carvery for lunch. Fela appeared happy and smiled. We enjoyed the meal and Fela had dessert afterwards. Fela was then congratulated on his educational achievement with Liz and we returned home. On arrival, Fela relaxed in the lounge and watched ‘Despicable me’. Later, he was observed dozing, so he was offered a nap and slept for about 10 minutes. Kingsley arrived for the evening shift and a handover was provided. Afterwards, Fela enjoyed his dinner. We then did a walk at home with Brian and Fela participated well. Fela was  offered a shoulder massage and had his shave, followed by his snack and bath. Fela remained in a good mood and appeared settled throughout the day.

The fan was moved away as the temperature in Fela’s bedroom was good to sleep without it. The 1 tog duvet was maintained, music on low and he wore vest and underwear. He passed urine, brushed his teeth, gurgled with the mouthwash and he was in his bedroom at 22:20, the evening routine was done and he was in bed at 22:35 after prayer. He slept off at 22:42, first wake at 2:40, he went to pass urine so I can check his bed, bed dry and after visiting his bedroom I felt he needed the fan as the room was slightly warm. I kept the fan on low for the rest of his sleep, he slept at 3:40 and woke up at 5:58, he lied down quietly, playful and I went to him at 6:30. We came to start the morning routine, he visited upstairs again to tread before coming downstairs to have his supplements. He passed urine after having his smoothie and I assisted him to shower. After shower he dried himself, brushed his teeth, gurgled and then wore an underwear, we said prayer and he treaded before starting to eat his breakfast and had his morning supplements. He got clothed, we returned to the lounge, handed given to MH & OP. The kefir seeds were not separated as they are not enough to do so.

Came for evening duty.  Brandy gave handover , Fela was smiling and relaxing. Mina was preparing dinner. Later I gave  him his supplements. Once dinner was  ready we sat, said prayer and we ate, followed by water and his hot choc drink. After, he dried the dishes and then sat relaxing in lounge.  Fela did walk at home with Brian. I did the kefir milk. Fela’s been in a very good mood all evening and no sign of sleepiness . He later had his snack, sleep tea and supplements before ringing mama, after talking, said bye.  When done had a shave.  Then had last supplement. After had his bath, assisted by Mina. Splashing  in bath at time of report.

I assisted Fela out and dried, then he  brushed his teeth, used mouthwash with smiley cup then used  the toilet, after we went to bedroom, moved items, walked up and down corridor, then settled, we then went through the night routine.  Was 22.3 in the room, felt normal, 15 outside.  Fela then got  in bed with 1 tog duvet, said prayer, gave a  massage,  music on low,  fan on low. Left at 10.20pm. Asleep by  10.30pm. Slept  until waking  at 02.00am, I asked  Fela to use the toilet, he did and I checked, bed was dry. Got back to bed 02.05am. Back to sleep at 03.20am. Fela awoke at 06.15am, asked Fela to use the toilet, did so . After moving items and back we then went downstairs, gave Fela waking water , Fela then had his supplements with water, and after got in the shower. Music put on. Assisted out and dried. Once ready fela brushed his teeth, had mouthwash, after he moved various items in shower room, then again moving items in dining room eventually we sat, said prayer and drank his smoothy before  eating breakfast and he then had his water and supplements . We went upstairs and we went through the morning routine. After we came downstairs and relaxed in the lounge with music.  Mina and Dan  came on morning shift. I handed over. When leaving Fela was relaxing in the lounge.

arrived for day duty. Fela had completed his morning routine and was vocal, although he did not appear unhappy. I greeted him and he responded with a thumbs up.A social story about a visit to the optician and a walk at Birchall Playing Field was printed and read to Fela. He accepted and we got ready. Carla then came to pick us up. We first attended Fela’s optician appointment, where he did extremely well. He remained calm and cooperative throughout the eye test. Afterwards, we drove to Birchall Playing Field, where we had an enjoyable time. We played football together and went for a walk around the field. Fela did well with kicking the ball and scored three goals. We then completed one full round of the park before returning home. Upon returning home, Fela had his lunch and dessert. Afterwards, he sat in the lounge listening to music. He appeared tired and was observed dozing off, so he was offered a nap. He slept for about 30 minutes. When he woke up, Fela declined Makaton but agreed to education at home (drawing). He was initially supposed to draw an elephant. However, rather than looking at the elephant for drawing, he used his imagination and drew about 20 pictures that resembled knives. The drawings  looked beautiful. Afterwards, Brandy read Fela a story,( the ugly duckling)which he appeared to enjoy. Marc arrived for evening duty and a handover was provided. Fela enjoyed his dinner and remained in a happy mood. We did an 18 minute walk at home with Brian, after which Fela was offered a shoulder massage. He then had his snack and was assisted to phone Carla.Later, Fela had his shave and bath.

In the morning upon arrival for my shift BK gave myself and KO handover. CC also gave us instructions and it was followed accordingly. Social story was read to Fela then his board was updated. All necessary things were packed in his bag and prepared him for the shopping.

His broth was given to him then he pushed the trolley for the shopping. Fela also used the washroom during after the shopping. We then drove to Tunstall park for 46 minutes walk then after we drove to Green pastures for Fela’s almond milk the drove back home and we quickly fixed lunch as Fela was hungry. After that  we unpacked the shopping. After lunch Fela had 30 mins of nap. He woked up and we served him with his snack. We couldn’t do education and makaton because time was far spent. Later BK arrived and KO gave her handover. Fela’s dinner was being prepared. He had his supplements then after we served him with dinner. After dinner Fela refused to wipe the dishes as he said he wants to watch TV. He came to the lounge and his music was on and he was enjoying it. Fela refused the neighbourhood walk and also refused fitness with Brian. He had his shoulder massage. Later we offered him his supplements, snack and sleep tea and gave him shave afterwards. I took Fela to the bath and assisted him. He was still in the bath splashing water at the time of this report.
