05/06/2025 – Sleep shift – Mercy Kemboi

Shift Report:

Personal care was completed with reassurance and verbal prompts where needed The evening routine was completed smoothly and on schedule. Fela was calm and cooperative throughout, and he was supported in preparing for bed. He was in bed by 10:14 p.m. and appeared relaxed. Fela fell asleep at approximately 10:25 p.m. and remained asleep without any disturbances through the night. There were no signs of restlessness, vocalisations, or discomfort observed during sleep.

At 5:26 a.m., Fela woke up naturally. I encouraged him to use the toilet, which he did without resistance. I also checked his bed for any signs of wetness; the bedding was clean and dry. After returning to his bedroom, I assisted him in putting on his eye mask to help him settle back into bed. Although he remained quiet and comfortable, he did not return to sleep.

A short time later, Fela got out of bed and came downstairs. He was calm and alert. He took his awake supplements as prescribed and was then supported with a warm shower, which he tolerated well.Following his shower, I served him breakfast, which he ate without issue. He then took his morning supplements as per protocol.

After breakfast, Fela returned to his room, where personal care was completed with reassurance and verbal prompts where needed. He was encouraged to choose what he wanted to wear, and he did so amazingly—making confident and appropriate choices independently. He was engaged throughout and responded well to prompts, requiring only minimal assistance.

When the day staff arrived, I provided a handover, detailing Fela’s overnight rest, morning activities, and overall presentation. Petty cash was reviewed and signed over accordingly. Fela remained in good spirits and transitioned smoothly into the care of the day team.
