Shift Report:
Was giving sleep mix 9:15pm and was out of the bath by 9:18pm
We went upstairs for his evening routine and had his head massage and was in bed by 9:46pm and slept around 9:55pm he woke up at 2:22am prompted him to use the toilet, he came back to bed and did not sleep again, around 6: 12am he was served with his supplement and tea. I assisted him to have his shower and morning routine.
