Fela was supported with his evening routine and was in bed by 10:07 p.m. He settled well and fell asleep by approximately 10:15 p.m.
He woke at 11:40 p.m. and was encouraged to use the toilet, which he did. I went to his room afterward to check if the bed was wet; it was dry. I then supported him to put on his eye mask, but he remained awake.
At 2:04 a.m., Fela was prompted again to use the toilet. He returned to bed and fell asleep around 3:00 a.m., but only slept for about 30 minutes before waking once more to use the toilet. He was guided back to bed and remained there, though he was vocal at times. At 03:26a.m., I encouraged him to use the toilet again, but he declined.
At 6:15 a.m., since he was already awake, I offered him a glass of water. He later came downstairs to begin his morning routine, had breakfast, and returned to his room to complete his personal care.
Day staff arrived, and I handed over the shift and signed the petty cash book.
Fela had a restless night, with frequent awakenings and limited continuous sleep.
