Shift Report:
Was giving sleep mix 9:41pm and was out of the bath by 9:50pm
We went upstairs for his evening routine and had his head massage and was in bed by 10:14pm and slept around 10:40pm he woke up at 2:52am prompted him to use the toilet after he went back to bed and did not sleep again, he was vocal , went upstairs to check the bed , it was not wet ,at 6:05am he was served with his supplement and tea. I assisted him to have his shower and morning routine.
